By submitting this application, I declare that:
1. The information I have given is true, complete and accurate.
2. Every document I have uploaded is genuine and has not been altered.
3. I am the Superintendent Pharmacist designated to oversee this electronic pharmacy, or I am authorised to apply on their behalf, and I understand that a Superintendent Pharmacist may register only one electronic pharmacy (regulation 34(4)).
4. I have read the Electronic Pharmacy Regulations, 2026 that apply to this application.
5. I have read the Terms of Use, the Privacy Policy and the Payment Policy, including how my information is processed.
6. I understand the fees that apply, and that paying a fee does not guarantee registration, approval or a licence.
7. I understand that the Council may inspect, request corrections, and approve or refuse this application under its procedures, and that I must follow them.
8. I understand that making a false statement or misrepresenting information to obtain registration or licensing is subject to an administrative fine of N500,000.00 under regulation 39, and may lead to suspension, revocation or withdrawal of a licence under regulation 14.
